Regardless of an individual's back story the Homeless Adult Team (HAT) has a "whatever it takes" motto to help its clients reach recovery. The team provides intensive behavioral health and co-occurring treatment and services to adults who have been or are at risk of being homeless, in psychiatric hospitals, in prison or on probation over the past year.
Once someone is referred, the team assesses each individual and works together to determine the best treatment plan to meet their specific needs. The support doesn't end with the treatment plan.

Case Management
The Homeless Adult Team's focus is to help each person build their confidence and set the goals they want to accomplish in their recovery. Case managers work directly with clients to link them to community resources. They also host various groups and provide home visits to engage clients and teach them coping skills.
Peer Support
There's no better way to navigate the unknowns of recovery than working with someone who's been through the process themselves. That's where HAT's peer support specialist comes in. Peers are paraprofessional staff who use their own lived experience to guide and support a client through their treatment and recovery journey.
